Job description:
- Conduct thorough assessments of patients' health conditions in their home environments.
- Develop personalized care plans and collaborate with multidisciplinary teams to deliver optimal patient care.
- Administer medications and treatments as prescribed by the physician.
- Provide education and support to patients and their families on disease prevention and health maintenance.

Qualifications:
- Current Registered Nurse (RN) license in the state of Minnesota.
- Minimum of 1-2 years of experience in a home health setting.
- Strong assessment and clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently and manage time effectively in a home health environment.
