Job description:
- Conduct comprehensive physical assessments and review medical histories to evaluate patients' conditions.
- Create and implement personalized treatment plans tailored to each patients unique needs and objectives.
- Administer therapeutic exercises and training programs to enhance patient recovery.
- Provide education to patients and their families on injury prevention, wellness, and the recovery process.
- Monitor, document, and report on patients' progress towards their rehabilitation goals.
- Collaborate with multidisciplinary healthcare teams to ensure comprehensive patient care.
- Maintain accurate and compliant patient records and documentation.
Qualifications:
- Masters degree in Physical Therapy.
- Minimum of 1 year of experience in home health as a physical therapist.
- Current physical therapy license within the state of practice, adhering to professional standards and practices.
- Valid CPR certification, maintained throughout the clinical tenure.
- Licensed driver with the ability to travel to various business locations.
