Key Responsibilities:
- Manage and oversee patient care services, ensuring compliance with all regulatory standards.
- Lead, mentor, and support nursing staff, fostering professional growth and enhanced patient outcomes.
- Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
- Ensure accurate and timely documentation of patient care activities and outcomes.
- Conduct regular audits of patient records and care plans to uphold quality assurance.
- Act as a liaison among patients, families, and healthcare providers to ensure seamless care coordination.
- Engage in quality improvement initiatives and participate in ongoing professional development.
Qualifications:
- A minimum of two years' experience in a management or supervisory role within a home care setting is preferred.
- Strong knowledge of professional standards, Medicare Conditions of Participation, and compliance with federal, state, and local regulations.
Required Certifications and Licensures:
- Current licensure as a registered nurse in the state of practice.
