Job description:
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our home health team in Waco. This full-time position offers an exciting opportunity to work with patients in their homes, providing personalized care and helping them regain their independence and improve their quality of life.
Responsibilities:- Conduct thorough patient assessments to identify needs and create individualized treatment plans.
- Implement therapeutic exercises, manual therapy techniques, and other interventions to address patient goals.
- Educate patients and their families on exercises and activities to improve mobility and function.
- Monitor patient progress and adjust treatment plans as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
- Maintain accurate and timely documentation of patient care and progress.
Qualifications:
- Current state licensure as a Physical Therapist.
- Proven experience in a home health setting is preferred but not required.
- Strong interpersonal and communication skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
