Physical Therapist -- We are seeking a highly skilled Physical Therapist to help improve the quality of life for residents at a skilled nursing and rehabilitation facility in Hornell, NY.

- As a Physical Therapist, you will be responsible for evaluating and treating residents to help them achieve long-term health benefits and improve their overall mobility and quality of life.
- Evaluate residents' conditionsand develop individualized physical rehabilitation plans.
- Help residentsimprove mobilityand functional independence.
- Relieve painusing exercises, hands-on therapy, stretching, and physical therapy devices.
- Assist residentsin using physical therapy equipment effectively.
- Guide residents through therapy tasks, monitoring theirperformanceand progress.
- Inform residentsabout what they should expect at different stages of their recovery.
- Supervise physical therapy assistantswhen required.
- Maintain asafe and clean working environment.
- Adhere to infection-control policiesand protocols.
- Contribute tointerdisciplinary team effortsto ensure comprehensive resident care.
- Graduate of an accredited Physical Therapy Program.
- Current New York State PT License.
- Experience in skilled nursing/long-term care settings preferred.
