Job description:
Responsibilities:
Manage daily Key Performance Indicators (KPIs), including monitoring productivity, visit utilization, and quality metrics.
Provide clinical and operational support to field staff (RNs, therapists, LVNs, and aides), including troubleshooting, guidance, and escalation of issues.
Process verbal orders, coordinate workflow, and handle escalation or complaint calls efficiently.
Support case management by reviewing and managing patient episodes, with focus on preventing Low Utilization Payment Adjustment (LUPA) risks, appropriate paraprofessional (HHA) utilization, correct visit types, and overall episode management.
Report on case management activities to ensure quality outcomes and regulatory compliance.
Update clinical records, assist with documentation, and maintain accurate patient information in the Electronic Medical Record (EMR) system.
Coordinate with clinicians, interdisciplinary team members, and other departments to promote continuity of care and efficient operations.
Participate in quality improvement initiatives, case conferences, and performance reviews as needed.
Assist with care plan reviews, missed visit management, and other administrative/clinical support tasks to optimize patient outcomes and agency performance.
Qualifications:
Qualifications:
- LVN/LPN with current licensure to practice nursing in the practicing state.
- Possess and maintain CPR certification.
- Previous experience in home care setting preferred.
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order.
- Prior use of Electronic Medical Records (EMR) preferred.
