Job description:
Responsibilities:
Patient Assessments: Conduct comprehensive physical, emotional, and psychosocial assessments.
Care Planning: Develop and evaluate individualized care plans alongside interdisciplinary teams.
Skilled Nursing: Provide direct care, including wound management, IV therapy, and symptom/pain control.
Family Support: Deliver tailored education and emotional support to patients and caregivers.
Team Supervision: Oversee LPNs, home health aides, and support staff.
Urgent Response: Manage patient admissions, urgent clinical needs, and on-call rotations.
Condition Monitoring: Track patient changes and dynamically update care plans.
Compliance & Records: Maintain precise clinical documentation in alignment with agency standards and regulations.
Qualifications:
RN License: Active state RN license (unencumbered).
Experience: Minimum 1 year of RN experience.
Certifications: Current CPR certification.
Transportation: Valid drivers license, reliable personal vehicle, active auto insurance, and willingness to travel locally.
